Driver's Eligibility Information

Please note that you will need to make an appointment to come in during June/July. We are closed the week of July 4th

1) Pick up a Driver's Eligibility form from our office. If needed, we can mail the form to your parent/guardian with a stamped, return envelope to mail the signed form back to us.

2) Have a parent/guardian sign the form.

3) Bring the signed form to our office along with a copy of your passport or birth certificate and we will sign and seal the form.

4) Go pass the test and be safe out there!

PLEASE NOTE: The Driver's Eligibility Certificate is an official NC State document and cannot be emailed or copied.
